APPOINTMENTS We see patients by appointment
only. We accept new
patient referrals from other physicians
and self-referrals for continuation of care or second opinions.
New patient visits are scheduled for 30-60 minutes.
It is extremely important that you bring copies of all necessary
records, X-rays, pathology reports and laboratory studies to the initial
visit. Please request that
your referring physician mail records at least one week in advance, fax
them to our office, or give them to you to hand carry.
If our office has mailed information forms to you in advance, please
bring the completed forms with you at
the time of your appointment.
Please be sure to bring your insurance cards with you. Return visits are usually
scheduled for 15 minutes. If
you think your visit will require more than the scheduled time, (e.g.,
a family conference or major change in your condition), please notify
our office so that additional time can be scheduled.
Please notify our office as soon as possible if you will be late
or if you will be unable to keep your appointment. Late
patients may be asked to reschedule. AAHOA
physicians also see
patients in the Woodland Health Center in Brighton. Most, but not
all, chemotherapy treatments can be administered at Woodland.
All
calls regarding appointments or treatments at Woodland should be addressed
to our main office in the St. Joseph Mercy Cancer Care Center (734-712-1000).
All calls requiring advice
from the physician or nurse should also
be directed to the main office. If
you have registered and obtained your patient identification number (PIN),
you may use this online form to schedule routine revisits, with your physician.
All areas marked with an asterisk (*) must be completed. Do not use this
form if you are having problems or if you need an urgent appointment (within
2 weeks).
